Job Description:

We are seeking a compassionate Recovery Oriented Nurse to join our Assertive Community Treatment (ACT) team at Monadnock Family Services. As a member of our multidisciplinary team, you will work closely with patients, families, and healthcare professionals to provide holistic care and support services.

The ideal candidate will have a strong background in mental health nursing, excellent communication skills, and the ability to work effectively in a fast-paced team environment. A Master's degree in nursing or equivalent is required, along with a valid driver's license in the state where the team operates.

Key Responsibilities:

Your primary responsibilities as a Recovery Oriented Nurse will include:

  • Providing direct care and support to patients in their homes and communities.
  • Collaborating with the ACT team to develop and implement individualized treatment plans.
  • Conducting regular home visits to monitor patient progress and provide emotional support.
  • Facilitating communication between patients, families, and healthcare providers to ensure seamless care coordination.
Salary and Benefits:

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience, plus comprehensive benefits including health insurance, dental insurance, flexible spending accounts, paid time off, and 403b retirement plan.

Requirements:

To be considered for this role, you must have:

  • A Master's degree in nursing or equivalent.
  • At least one year of experience in a mental health setting.
  • A valid driver's license in the state where the team operates.
