JOB PURPOSE: The primary aim is to enhance the well-being of patients and their families by overseeing patient care processes, which include assessment, planning, implementation, and evaluation in partnership with the patient, family, physician, and relevant support services tailored to the specific age group served.

ESSENTIAL JOB FUNCTIONS

1. Coordinates patient assessments through comprehensive health data collection.

Performance Standards:
• Prioritizes data collection based on the patient's immediate condition or needs.
• Conducts ongoing and systematic comprehensive physical and psychosocial assessments.
• Involves the patient, significant others, and healthcare providers as appropriate.
• Performs functional, nutritional, and other screenings, ensuring completeness.

2. Analyzes patient assessments to establish nursing diagnoses and identify expected outcomes.

Performance Standards:
• Determines nursing diagnoses using assessment data.
• Identifies patient outcomes based on nursing diagnoses.
• Prioritizes outcomes based on the patient's condition and needs.

3. Coordinates an interdisciplinary care plan that prescribes interventions to achieve expected outcomes.

Performance Standards:
• Collaborates with relevant disciplines to formulate an individualized care plan based on patient needs and current nursing standards.

4. Implements the interventions outlined in the care plan.

Performance Standards:
• Collaborates with the healthcare team to deliver direct patient care according to nursing standards.

5. Evaluates the patient's progress towards achieving outcomes.

Performance Standards:
• Conducts ongoing and systematic reassessments based on departmental standards.

6. Serves as the primary educator for patients and staff regarding wound, ostomy, and continence care.

Performance Standards:
• Educates patients and staff on W.O.C. needs to facilitate rehabilitation through independence and self-care.

EXPERIENCE REQUIREMENTS: Minimum of 1 year of clinical RN experience required.

EDUCATIONAL REQUIREMENTS: Graduation from an accredited WOCN program preferred; WOCN certification within 18 months of employment.

SPECIAL SKILLS, LICENSE, AND KNOWLEDGE REQUIREMENTS: Current RN licensure in Louisiana.

HIPAA REQUIREMENTS: Adheres to all applicable HIPAA regulations relevant to the position.

SAFETY REQUIREMENTS: Maintains knowledge of and compliance with all relevant safety practices for the position.
