    • Encourage independence and autonomy in daily life, promoting self-advocacy and self-determination.
  • Community Involvement
    • Facilitate opportunities for individuals to participate in their community, promoting socialization and integration.
    • Support individuals in developing skills for community living, including budgeting, money management, and self-preservation.
  • Training and Support
    • Provide training and instruction in activities of daily living, community recreation, and personal hygiene.
    • Support individuals in developing skills for employment, education, and personal growth.
  • Documentation and Communication
    • Document services and instruction in daily and monthly progress notes, ensuring accurate and timely reporting.
    • Communicate individual needs and progress to the Coordinator or Assistant Director as needed.
  • Performance and Development
    • Achieve minimum caseload and service delivery requirements, ensuring high-quality support and services.
    • Participate in the development and review of the Community Habilitation Staff Action Plan, contributing to the growth and development of the team.
  • Behavioral Support
    • Accurately record and submit behavioral data in a timely manner, ensuring accurate tracking and reporting.
    • Participate in additional training and professional development as required, staying up-to-date on best practices and industry standards.
Requirements:
  • High School Diploma required.
  • One-year combined work, volunteer, internship experience with people with developmental disabilities.
