Community Habilitation Specialist

We are seeking a highly motivated and dedicated Community Habilitation Specialist to join our team at Summit Educational Resources. The successful candidate will be responsible for providing person-centered programming and support to individuals with developmental disabilities, enabling them to make informed decisions and participate in their community to the fullest extent possible.
Key Responsibilities:
- Develop and implement individualized plans to promote independence and community participation
- Provide training and instruction in daily living skills, community recreation, and personal hygiene
- Document services and progress in daily and monthly notes
- Collaborate with the Coordinator and Assistant Director to achieve minimum caseload and service delivery requirements
- Participate in the development and review of the Community Habilitation Staff Action Plan
Requirements:
- High School Diploma required
- One year of combined work, volunteer, or internship experience with individuals with developmental disabilities
