Job Summary:

The Social Worker provides comprehensive psychosocial assessments to members, supporting their ability to direct their own healthcare experience through assessment, intervention, education, and linkage to health plan and community resources.

Essential Responsibilities:
  • Utilize age-appropriate assessment and intervention skills to address patient needs.
  • Assist members in developing cost-effective, appropriate healthcare plans by optimizing personal resources, community resources, and Kaiser Permanente services.
  • Promote patient-centered, multi-disciplinary care that considers personal, psychological, economic, and cultural needs.
  • Maintain thorough knowledge of Kaiser Permanente services, benefits, public and private community resources, and federal and state laws affecting healthcare planning.
  • Complete required documentation of services and Social Work plan of care according to department policy.
  • Participate in program development, orientation, and educational activities to enhance members' capacity for self-directed healthcare.
Basic Qualifications:
  • Experience: Minimum one year of Social Work experience in a hospital, medical outpatient clinic, home health/hospice, or long-term care setting, or successful completion of the Kaiser Permanente NW Social Worker Internship program.
  • Education: Masters degree in social work from an accredited university.
  • License, Certification, Registration: This job requires credentials from multiple states. Credentials from the primary work state are required before hire. Additional Credentials from the secondary work state(s) are required post-hire.
Additional Requirements:
  • Ability to work effectively on a multi-disciplinary team.
Preferred Qualifications:
  • Minimum three years of professional Social Work experience in a hospital, medical outpatient clinic, home health/hospice, or long-term care setting.
  • Working knowledge of KPNW services and benefits.
  • Expertise with short-term counseling interventions.
  • Demonstrated ability to provide leadership to a multi-disciplinary team.
  • Advanced knowledge of local and national resources related to healthcare.
