About the Role

We are seeking a skilled and compassionate Home Health Care Nurse Specialist to join our team. In this role, you will be responsible for providing high-quality patient care in the comfort of their own homes.

Responsibilities
  • Implementing physician-prescribed plans of care for patients
  • Conducting patient assessments and coordinating care with other healthcare professionals
  • Administering prescribed medications, treatments, and therapies as needed
  • Maintaining accurate and timely documentation of patient care in our electronic system
  • Providing health education and training to patients and their families
Requirements
  • An active, unencumbered nursing license in the state where you will practice
  • CPR certification from the American Red Cross or American Heart Association
  • A TB skin test (within the last 12 months)
  • At least six months of prior hands-on nursing experience
