Key Responsibilities:
- Develop and implement transition plans in coordination with patients, families, and interdisciplinary teams.
- Provide education and support to patients and families to enable them to progress towards care goals.
- Collaborate with healthcare teams to ensure timely and effective care transitions.
- Apply clinical expertise and knowledge of nursing, care management, and community resources to establish transition plans.
Requirements:
- Nursing degree or diploma required upon hire.
- Bachelor's degree in Nursing or higher within 3 years of hire.
- Oregon Registered Nurse License upon hire.
- 2 years of acute care hospital or community-based experience.
- 1 year of experience with electronic medical records or documentation systems.
- 1 year of Home health, mental health, substance use, hospice, utilization review, discharge planning, or care management experience.
