Job Summary

We are seeking a skilled and compassionate Nurse Coordinator to join our team at Element Care. As a key member of our enrollment team, you will play a vital role in ensuring the successful enrollment of potential participants into our PACE program.

Key Responsibilities
  • Conduct initial appointments to introduce potential participants to our PACE model and assess their eligibility for our program.
  • Utilize a strength-based approach to evaluate the current needs and health status of potential participants, ensuring they meet the necessary criteria for enrollment.
  • Communicate effectively with our interdisciplinary team (IDT) regarding the enrollment status and progress of prospective participants.
  • Facilitate collaboration between prospective participants, caregivers, and referral sources to ensure a smooth enrollment process.
  • Participate in weekly enrollment team meetings to provide clinical insights and support in addressing challenging referrals and other barriers.
  • Maintain accurate data on new referrals in our CRM, EMR software, and Masshealth Virtual Gateway.
  • Coordinate with our marketing and outreach team to assist with outside events.
Requirements
  • Associate's or Bachelor's degree in Nursing or completion of a Nursing Diploma Program.
  • Current licensure as a Registered Nurse in the Commonwealth of Massachusetts.
  • Minimum one year of experience in a geriatric healthcare and/or adult mental health setting.
  • Advanced computer skills, including Microsoft Office, Customer Relationship Management, and Electronic Medical Records software.
  • Ability to communicate effectively with diverse medical professionals and establish working relationships.
  • Strong knowledge of medical and psychiatric diagnoses and treatments.
  • Experience working with individuals from diverse socio-cultural backgrounds.
  • Ability to assess multiple perspectives and priorities, facilitating solutions within a goal-oriented framework.
