About the Role

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team at Quality Talent Group. As a key member of our home health care team, you will play a vital role in providing high-quality patient care and support to our clients in the comfort of their own homes.

Responsibilities
  • Deliver High-Quality Patient Care: Provide skilled nursing care to patients in their homes, under the supervision of a Registered Nurse, in accordance with the developed care plan and physician orders.
  • Assess and Coordinate Care: Continuously observe and assess patient condition and care needs, and report changes in condition to the supervisor and/or physician as appropriate.
  • Document Care Provided: Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner whenever care is delivered.
  • Participate in Care Planning: Participate in the development and implementation of the nursing care plan, and update as necessary.
  • Provide Education and Support: Educate patients and families regarding the disease process, self-care techniques, and prevention strategies, and provide support to meet their nursing needs.
Requirements
  • Education and Licensure: Graduate of an accredited school of nursing, and current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Certifications and Experience: Current CPR certification, and demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures. Six months of recent experience as a Licensed Nurse in a clinical care setting is preferred.
  • Physical and Environmental Requirements: Must be able to speak, write, read, and understand English, and have visual and hearing acuity. Must be able to lift 50 pounds, and have strong sense of smell and touch. Must be able to function in a wide variety of environments, which may involve exposure to allergens and other various conditions.
