About the Role

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team at Quality Talent Group. As a Private Duty Nurse, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Deliver Quality Patient Care: Provide skilled nursing care to patients in their homes, under the supervision of a Registered Nurse, in accordance with the developed care plan and physician's orders.
  • Assess and Coordinate Care: Continuously observe and assess patient condition and care needs, reporting changes in condition to the supervisor and/or physician as appropriate.
  • Document Care: Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner whenever care is delivered.
  • Participate in Care Planning: Participate in the development and implementation of the nursing care plan, taking into account the patient's medical history, current condition, and treatment goals.
  • Communicate Effectively: Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination and patient satisfaction.
Requirements
  • Education and Licensure: Graduate of an accredited school of nursing and current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Certifications: Current CPR certification and demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
  • Experience: Six months of recent experience as a Licensed Nurse in a clinical care setting, with a preference for home health experience.
