Key Responsibilities- Care Coordination

- Monitor patients' progress and adjust care plans as needed to ensure optimal outcomes.
- Stay up-to-date on the latest clinical guidelines, research, and best practices in patient care.
- Discharge Planning
- Conduct thorough assessments of patients to identify their discharge needs and develop effective plans to meet those needs.
- Coordinate with patients, families, and healthcare providers to ensure a smooth transition from hospital to home or other post-acute care settings.
- Identify and address any barriers to discharge, such as lack of transportation or equipment, and develop solutions to overcome these challenges.
- Utilization Management
- Review patients' medical records to ensure that their care is medically necessary and meets established standards of care.
- Collaborate with payors and other stakeholders to ensure that patients receive the care they need in a timely and cost-effective manner.
- Identify opportunities for process improvement and implement changes to optimize patient care and reduce costs.
- Education
- Graduate of an accredited program in nursing or a related field.
- Master's degree in nursing or a related field preferred.
- Licenses/Certifications
- Current RN licensure in the state of Colorado.
- Certification in case management or a related field preferred.
- Experience
- At least two years of experience in a healthcare setting, preferably in a case management or utilization review role.
- Knowledge/Skills/Abilities
- Knowledge of government and non-government payor practices, regulations, standards, and reimbursement.
- Knowledge of Medicare benefits and insurance processes and contracts.
- Knowledge of accreditation standards and compliance requirements.
