Job Summary

We are seeking a skilled Registered Nurse to join our team as a Hospice RN Case Manager. In this role, you will provide direct patient care services, act as a liaison between physicians and patients/families, and empower caregivers through education and support.

Key Responsibilities:
  • Provide direct patient care services to hospice patients and supportive care to patients' families
  • Act as primary liaison between physician, patient/family, and hospice team
  • Empower patient's caregivers through education and support
  • Manage and collaborate with interdisciplinary team to provide comfort, peace, and dignity
  • Evaluate patient and family needs and assist in preparation and execution of plan of care
  • Maintain all assigned patient clinical records
  • Participate in company quality assessment and performance improvement programs
Requirements:
  • RN license in the state of requested employment (or eligibility to obtain)
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
  • Current driver's license and automobile insurance
  • Current CPR certification
