Job Summary:

Critical Connection, inc is seeking a skilled Physical Therapist to join our team at a Skilled Nursing Facility in a travel assignment. As a Physical Therapist, you will be responsible for providing high-quality physical therapy services to patients in a geriatric population.

Responsibilities:

  • Develop and implement individualized physical therapy plans to meet the unique needs of each patient
  • Conduct thorough assessments and evaluations to identify patient needs and goals
  • Provide physical therapy interventions, including exercises, modalities, and education, to promote patient independence and mobility
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care
  • Document patient progress and outcomes in accordance with facility policies and procedures

Requirements:

  • Master's Degree in Physical Therapy from an accredited program
  • Current Physical Therapy license in good standing
  • Experience working in a Skilled Nursing Facility or similar setting
