We are seeking a highly skilled Early Childhood Case Manager to join our team of dedicated professionals. This role will play a vital part in supporting the well-being of families with children aged birth to 6.

Job Description:
  • Provide case management services for at least 30 open cases, coordinating services and supports for each family member to ensure their health, wellness, and safety.
  • Maintain case ownership responsibility, continuously monitoring and assessing family dynamics to make necessary assessments and coordinate supports.
  • Develop and implement individualized family support plans, working closely with parents/caregivers to achieve their goals.
  • Provide timely referrals to services, building strong relationships with community resources and providers.
  • Collaborate with families to strengthen parent-child relationships and support networks, promoting healthy development and growth.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with diverse populations. A Bachelor's Degree in Social Work or a related field is required, with experience in primary responsibility for providing case management services to families with young children.

Salary: $45,000 - $60,000 per annum, depending on qualifications and experience.
