Job Title: Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Jackson Healthcare Government Services, LLC. As a Clinical Pharmacist, you will play a critical role in providing pharmaceutical care to patients in a primary care setting.

Key Responsibilities:
  • Provide clinical pharmacy direction for all clinical activities within the Medical Treatment Facility (MTF).
  • Collaborate with the Home Port Team to monitor Healthcare Effectiveness Data and Information Set (HEDIS) measures and intervene in patients with chronic disease states.
  • Review emergent/urgent care and admission consults to monitor trends, review medication treatment, and follow up as indicated.
  • Perform telephone follow-ups to assess new medication regimens as requested by providers.
  • Assist with a medication refill program to expedite refill processes and increase the Primary Care Provider's time for direct patient care.
Requirements:
  • Current, full, active, and unrestricted license to practice as a pharmacist.
  • Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy or documentation of appropriate education, training, and/or CME in the practice of clinical pharmacy.
  • One year of full-time experience in the last three years as a clinical pharmacist in an ambulatory care setting or completed an ASHP accredited Ambulatory Care Residency or possess Board Certification as a Pharmacotherapy Specialist.
