Key Responsibilities
  • Collaborate with healthcare teams to monitor and manage chronic disease states, including asthma, diabetes, hypertension, and pain management.
  • Review emergent and urgent care consults to monitor trends and review medication treatment.
  • Ensure the quality and timeliness of records, reports, and documentation of services provided.
  • Provide clinical pharmacy direction for all clinical activities within the Medical Treatment Facility (MTF).
  • Assist with a medication refill program to expedite refill processes and increase the Primary Care Provider's time for direct patient care.
Requirements
  • Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
  • Current, full, active, and unrestricted license to practice as a pharmacist.
  • One year of full-time experience in the last three years as a Clinical Pharmacist in an ambulatory care setting, or completion of an ASHP-accredited Ambulatory Care Residency, or Board Certification as a Pharmacotherapy Specialist.
