Job Summary

We are seeking a skilled Dietitian to join our PACE Program team at UnityPoint Health. As a Dietitian, you will play a vital role in providing nutritional care to our participants and working collaboratively with the Interdisciplinary Team to ensure the best possible outcomes.

Key Responsibilities
  • Assess nutritional needs and develop personalized care plans for PACE participants.
  • Collaborate with the Interdisciplinary Team to implement and evaluate nutritional care plans.
  • Provide education and support to participants, family members, and healthcare professionals on nutrition and diet-related topics.
  • Monitor and maintain accurate records of participant information and care plans.
  • Participate in Interdisciplinary Team meetings and contribute to the development of care plans.
Requirements
  • Bachelor of Science degree in Dietetics or a related field.
  • Registration or registration eligible with the Commission on Dietetic Registration.
  • Licensed or licensed eligible with the State of Iowa.
  • Current CPR/BCLS certification.
  • Maintains current Adult Mandatory Reporter status.
  • Maintains American National Standards Institute accredited food protection/safety certification (i.e. ServSafe Food Protection Manager/Certified Food Safety Manager).
