Job Title: Dietitian PACE

Job Summary:

We are seeking a highly skilled Dietitian to join our PACE team. As a Dietitian, you will play a vital role in providing nutritional care to our participants, working closely with the Interdisciplinary Team to develop and implement personalized nutrition plans.

Responsibilities:

  • Assess nutritional needs and develop individualized nutrition plans for participants
  • Collaborate with the Interdisciplinary Team to ensure comprehensive care
  • Conduct nutritional assessments and evaluations as needed
  • Provide education and training to participants, family members, and staff on nutrition and food safety
  • Document participant information and nutrition plans in the electronic medical record
  • Attend Interdisciplinary Team meetings and participate in team activities
  • Coordinate meal planning and delivery for PACE Center meals
  • Monitor food safety and sanitation of PACE meal service

Requirements:

  • Bachelor of Science degree in Dietetics and/or food and nutrition
  • Registration or registration eligible with the Commission on Dietetic Registration
  • Licensed or licensed eligible with the State of Iowa
  • Current CPR/BCLS certification
  • Maintains current Adult Mandatory Reporter status
  • Maintains American National Standards Institute accredited food protection/safety certification
