We are excited to present a unique opportunity for a Per Diem Speech Therapist at Creative Solutions in Healthcare.

We are seeking a dedicated Speech-Language Pathologist to become part of our exceptional team of professionals who provide personalized care to our clients, ensuring positive outcomes.

Key Responsibilities:

  • Assess residents for swallowing difficulties or cognitive impairments.
  • Assist residents in achieving their goals related to cognitive and swallowing improvements.
  • Provide education to residents and their caregivers.
  • Conduct skilled, medically necessary speech and language therapy evaluations and create tailored treatment plans to enhance client independence and functionality.
  • Implement augmentative and alternative communication (AAC) systems for individuals facing severe expressive and/or language comprehension challenges.
  • Deliver training and guidance to clients, families, caregivers, and other professionals as a subject matter expert.
  • Collaborate with other professionals for diagnostic swallowing assessments as prescribed by physicians.
  • Exhibit proficient technical skills in accordance with current Speech Therapy standards.
  • Report any changes in residents' conditions promptly.
  • Maintain accurate and timely billing records along with supporting clinical documentation.
  • Address family inquiries and concerns in a timely manner.
  • Coordinate training with Restorative Nursing and other care staff.
  • Ensure the prevention of resident abuse or neglect.
  • Uphold confidentiality and protect all patient information.
  • Adhere to federal, state, and local legal and professional standards.

Essential Skills:

  • Experience in skilled nursing facilities or long-term care preferred.
  • Up-to-date knowledge of treatment methodologies.
  • Familiarity with Net Health Electronic Documentation System preferred.
  • Ability to effectively manage a diverse range of patient personalities.

Qualifications:

  • Active and valid Texas license in Speech-Language Pathology.
  • Current ASHA Certification.
  • In good standing with all regulatory bodies and licensing authorities.
