Job Opportunity

Corus Health is seeking a skilled Social Worker to provide essential support services to elderly home-bound patients and their families. As a member of our interdisciplinary team, you will play a vital role in delivering high-quality care and improving patient outcomes.

Key Responsibilities:

  • Provide counseling services to patients and families experiencing emotional, social, or economic challenges.
  • Assess patients' and families' social and emotional factors to identify areas of need and develop effective support plans.
  • Connect patients and families with community resources and services to promote independence and well-being.
  • Collaborate with healthcare professionals to ensure seamless care transitions and effective discharge planning.
  • Identify and address potential risks, such as suicide, abuse, neglect, and substance abuse, and provide appropriate referrals and support.

Requirements:

  • Master's Degree in Social Work from an accredited institution.
  • Licensed in New Mexico under the Social Work Practice Act.
  • Experience in healthcare or a related field, with a minimum of two years of supervised experience.
  • New Mexico driver's license and access to a reliable vehicle.
