About the Role

We are seeking a skilled Social Worker to join our team at DaVita, where you will play a vital role in supporting patients with End Stage Renal Disease.

Key Responsibilities
  • Provide emotional support and guidance to patients and their families in managing the physical, mental, and emotional demands of dialysis.
  • Help patients understand their rights and responsibilities, and facilitate their involvement in the care planning process.
  • Collaborate with the healthcare team to ensure comprehensive care and support for patients.
  • Develop and maintain relationships with patients, families, and healthcare providers to promote patient-centered care.

Requirements
  • Master's degree in Social Work (MSW) with a specialization in clinical practice.
