Position Overview:
As an Advanced Specialty Clinical Pharmacist, you will engage in a vital clinical role, contributing to the medication-use system while delivering tailored pharmaceutical care to patients within your designated areas.
Key Responsibilities:
- Provide comprehensive pharmaceutical care services, including assessing patient needs and integrating age and disease-specific factors into drug therapy and education.
- Adjust care based on patient responses and deliver clinical interventions to identify, mitigate, and prevent adverse medication events.
- Conduct clinical research and lead practice advancement initiatives aimed at enhancing medication-use processes and pharmacy practice.
- Offer medication and practice-related education and training, serving as a mentor for Doctor of Pharmacy students and pharmacy residents.
- Participate in quality management programs focused on improving services through process monitoring, data analysis, and intervention evaluation.
- Manage clinical care for patients undergoing various specialty therapies for conditions such as Hepatitis C, inflammatory diseases, pulmonary disorders, neurological issues, dyslipidemia, osteoporosis, and oncology.
Essential Duties:
- Facilitate pharmaceutical care services and medication utilization systems to align drug utilization with patient care needs and regulatory standards.
- Identify practice-related challenges requiring evaluation and lead clinical research or quality improvement initiatives as necessary.
- Ensure pharmacy's integral role in healthcare delivery and enhance pharmacy services and programs.
- Maintain proficiency in clinical pharmacy practices and identify high-risk patients to implement safety and quality improvements.
- Oversee prior authorization and appeals processes, providing clinical support to technician staff as needed.
- Document initial and follow-up assessments in accordance with program policies and accreditation requirements.
- Engage in pharmacy operations and medication dispensing as assigned, ensuring proficiency in operational programs and specialty pharmacy areas.
Qualifications:
A Doctor of Pharmacy degree from an ACPE accredited institution is required, along with either a PGY1 residency or five years of clinical practice experience in community, ambulatory care, PBM, or hospital settings. Experience in specialty pharmacy is preferred.
Familiarity with EPIC suite products is advantageous, alongside significant knowledge in advanced pharmacy practice and specialty medication therapy management.
Pharmacy preceptor license must be obtained within 90 days of employment.
