Program Overview:
The Pennsylvania Institute for Community Living (PICL) is a 15-bed modified therapeutic community residential program within Resources for Human Development, dedicated to providing integrated self-help and recovery-oriented treatment. Our program includes psychiatric rehabilitation, 24-hour staffing, and assistance in securing permanent housing. Participants typically engage in a 12-18 month journey to achieve their recovery-focused goals through structured lesson plans and group activities that enhance functional skills in areas such as social interaction, vocational training, financial management, and emotional coping.
Role Summary:
The Case Manager will assess and determine the resources necessary to address the unique needs of each participant. This role involves advocating for participants to ensure they receive essential services, identifying program needs, and recommending strategies to fill service gaps. A commitment to the core values of Resources for Human Development is essential in fulfilling job responsibilities. This position offers a competitive hourly wage of $18.50, with a Monday-Friday schedule from 10 am to 6 pm, allowing for some flexibility.
Key Responsibilities:
Collaborative Planning
- Participate as a member of a multidisciplinary team focused on delivering services from a holistic and values-driven perspective.
- Assist in creating and reviewing recovery plans in partnership with program participants, clinical staff, and other relevant stakeholders.
- Build and maintain relationships with community organizations to help participants access available resources.
Direct Client Engagement
- Deliver community-based services to program participants, emphasizing education, resource connection, community involvement, and achievement of recovery goals.
Administrative Duties
- Complete all required assessments and documentation as mandated by Resources for Human Development, funding bodies, and licensing/accreditation organizations.
- Perform additional tasks as assigned.
Additional Responsibilities:
- Evaluate and respond to the needs and service eligibility of individuals, both in-office (approximately 60% of the time) and in the community (approximately 40% of the time), demonstrating creative flexibility.
- Facilitate prompt screening and referrals to appropriate treatment and support services.
- Assist with the acquisition and coordination of various resources and benefits (e.g., Medicaid, Social Security, housing assistance) while fostering relationships with community agencies.
- Provide information and referrals for behavioral health and other human services, collaborating with necessary agencies.
Qualifications:
- Bachelor's Degree in Human Services or a related field.
- Intermediate proficiency in computer skills.
- Preferred: One year of Case Management experience.
- Required: Two years of general work experience supporting individuals in behavioral health, addiction recovery, or intellectual/developmental disabilities services.
