Key Responsibilities- Exemplary Practice and Outcomes
- Perform the Nursing Process (assessment, diagnosis, identification of outcomes, planning, implementation, and evaluation) in the performance of clinical care using evidence-based practice, analytical/critical thinking, and individualized care coordination.
- Provide relationship-based patient-centered care that adapts to population-specific characteristics, cultural diversity, and patient goals.
- Partner with patients and significant others to set and achieve care goals, using effective communication methods and timely documentation.
- Teamwork and Collaboration
- Coordinate the delivery and documentation of safe, quality patient care that promotes our professional care delivery model, including diverse and inclusive interdisciplinary communication methods.
- Demonstrate teamwork and collaboration by practicing fiscally responsible care and promoting the recruitment and retention of healthcare team members.
- Participate in performance improvement, research, and research utilization to promote safe, quality patient care, including initiating and leading activities and promoting shared governance.
- Professional Development and Initiative
- Complete initial and ongoing professional competency assessments, mandatory education, and population-specific education.
- Serve as a preceptor and/or mentor for other professional nurses and staff, ensuring a qualified workforce and modeling professional practice.
- Evidence-Based Practice and Research
- Promote evidence-based practice, nursing research, and performance improvement in nursing, including value analysis and decision-making processes.
- Uphold health care system/organizational policies, procedures, and clinical competencies, including legal, regulatory, and accreditation requirements and standards.
- Participate in data collection, pose clinical questions, and consult experts to address practice questions.
- Resources and Support
- Proactively plan for patient care across the care continuum, using direct patient care and collaboration with care managers and other healthcare team members.
- Participate in the development of protocols and procedures to achieve best practice outcomes, including reducing re-admission rates, avoidable days, and adverse events.
- Support efficient and effective use of human and material resources.
