Job Summary

We are seeking a skilled and compassionate Licensed Vocational Nurse (LPN) to join our Hospice Team in Indianapolis. As a key member of our interdisciplinary team, you will provide high-quality patient care, coordinate care with healthcare professionals, and promote a positive patient experience.

Responsibilities
  • Administer skilled nursing care to patients in their place of residence, under the supervision of a registered nurse.
  • Coordinate care with the interdisciplinary team, patient/family, and referring agency.
  • Provide effective communication to patients, families, and healthcare professionals.
  • Monitor assigned cases to ensure compliance with requirements of third-party payors.
  • Demonstrate commitment, professional growth, and competency.
Requirements
  • Graduate of an accredited school of vocational nursing.
  • One year experience as a Licensed Vocational Nurse in a clinical care setting required. Home health experience preferred.
  • Nursing skills as defined by generally accepted standards of practice.
  • Good interpersonal skills.
  • Reliable transportation.
Work Environment

This is a remote role with occasional meetings at an alternate location. You will work in patients' homes in various conditions, with possible exposure to blood, bodily fluids, and infectious diseases.

Physical and Mental Effort

Prolonged standing and walking are required, with the ability to lift up to 50 lbs and move patients. You will work under some stressful conditions to meet deadlines and patient needs, and make quick decisions and resource acquisition.
