As a Physical Therapist, you will play a crucial role in assisting patients to maintain, restore, and enhance their movement and functionality, ultimately improving their overall health and quality of life.
Working under a physician-approved care plan, your responsibilities will include delivering therapeutic interventions, conducting regular assessments, adjusting treatment plans as needed, and planning for patient discharge.
Position Type: PRN onlyCompensation:
$49.30 - $82.16 per hour
Key Responsibilities
1. Conduct comprehensive evaluations of patients during their initial visit following a referral.
2. Reassess patients at least every 30 days to ensure effective treatment.
3. Collaborate with physicians to revise care plans based on patient needs and document ongoing care requirements.
4. Provide physical therapy management, implementing interventions aligned with the approved care plan.
5. Maintain accurate documentation of each session in accordance with APTA guidelines and regulatory standards.
6. Participate in interdisciplinary team meetings, observing and reporting on patient responses to treatment and any changes in their condition.
7. Identify the need for additional services and facilitate appropriate referrals for comprehensive care.
8. Serve as a consultant to other healthcare providers as necessary.
9. Complete OASIS assessments accurately and in a timely manner.
10. Supervise Physical Therapy Assistants (PTA) in compliance with relevant regulations.
11. Ensure consistent oversight of patient care and remain accessible for communication with PTAs.
12. Assess PTA competencies and provide guidance as needed.
13. Document supervisory activities in the patient's medical records.
14. Engage in patient care conferences, quality improvement initiatives, and peer-review activities.
15. Uphold confidentiality regarding the therapist-client relationship.
16. Recommend follow-up care in coordination with the admitting care center and assist with community resource referrals.
17. Supervise Home Health Aides when nursing services are not active, ensuring they are informed of care plan changes.
18. Stay updated on physical therapy trends and best practices for service delivery and documentation.
19. Adhere to all policies, procedures, and regulatory requirements, including compliance with the Amedisys Compliance Program.
20. Perform additional related duties as assigned.
Qualifications
Required:
- Current, unencumbered license to practice physical therapy in the state of assignment.
- Graduation from an accredited physical therapy program recognized by the Commission on Accreditation of Physical Therapy Education (CAPTE).
- A minimum of one (1) year of experience in Home Health physical therapy.
- Current CPR certification.
