The Laurels of Bedford is seeking a dedicated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in providing medically-related social services to our guests, helping them to attain and/or maintain their physical, mental, and psychosocial well-being.
Responsibilities:
- Admissions and Social Services Evaluations: Complete thorough assessments of guests' needs, providing personalized social services or referrals as needed.
- Financial and Legal Support: Assist guests with financial and legal matters, such as applying for pensions, referrals to lawyers, and preplanning arrangements with funeral homes.
- Discharge Planning: Coordinate safe discharges by arranging waiting lists for community congregate living, home care services, and transfers to other facilities, working closely with outside agencies.
- Counseling and Psychiatric Services: Provide or arrange needed counseling services and psychiatric medication management as indicated.
- Documentation and Record-Keeping: Ensure accurate and timely documentation in MDS care plans.
Qualifications:
- Licensure and Education: Current social work licensure in the state and a Bachelor's degree in social work required; a master's degree preferred.
- Experience: One year of supervised social work experience in a healthcare setting is preferred.
