Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at Northern Light Health. As a Licensed Practical Nurse, you will be responsible for providing high-quality patient care to our patients in the home health setting.

Responsibilities:
  • Provide safe and effective patient care by using best practices during hand off communication and when reporting patient care information.
  • Collect and document subjective and objective systemic data according to established criteria.
  • Recognize and respond to abnormal diagnostic data and report to Registered Nurse and/or provider.
  • Develop and maintain a professional and personable relationship with patients/ families and staff.
  • Utilize critical thinking to prioritize and operationalize care.
  • Documentation is accurate and complete reflecting adherence to agency, regulatory and legal requirements.
Requirements:
  • Required: Licensed Practical Nurse
  • Required: High School Diploma/General Educational Development (GED)
Working Conditions:
  • Lifting, moving and loading 30 to 50 pounds.
  • Alternate shift schedules (day, evening, nights, weekends).
  • Potential exposure to diseases or infections.
