Job Summary:
As a Home Health RN, you will be responsible for administering skilled nursing care to patients in their place of residence, coordinating care with the interdisciplinary team, patients, and their families, and referring to other services as needed.
Key Responsibilities:
- Conduct initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of patients.
- Develop, initiate, and revise the plan of care as necessary to ensure quality and continuity of care.
- Coordinate services, inform the physician and other personnel of changes in the patient's condition and needs, and monitor assigned cases to ensure compliance with requirements of third-party payors.
- Prepare clinical and progress notes, complete appropriate documentation in a timely manner, and demonstrate commitment and professional growth by participating in in-service programs and maintaining or improving competency.
- Supervise, teach, and provide clinical direction to other nursing personnel, and promote the Agency's philosophy and administrative policies.
Requirements:
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Current State license as a Registered Nurse or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC).
- One year of experience as a Registered Nurse in a clinical care setting required, with home health experience preferred.
- Skills: nursing skills as defined by generally accepted standards of practice, good interpersonal skills, proof of current CPR, and reliable transportation and valid auto liability insurance.
