Job Overview:

We are seeking a seasoned Mental Health Practitioner to join our team at Momentum Resource Solutions. As a Mental Health Practitioner, you will be responsible for providing direct care and administrative oversight to our Assertive Community Treatment (ACT) team. This is an excellent opportunity for an experienced professional to make a meaningful impact in the lives of individuals and families struggling with mental illness and substance abuse.

Key Responsibilities:

  • Conduct comprehensive psychosocial assessments and treatment planning;
  • Provide evidence-based interventions in mental health and substance abuse, such as cognitive-behavioral therapy and harm reduction;
  • Assume administrative tasks, including chart auditing, collaborating with outside partners, and facilitating team meetings;
  • Supervise students/staff and assist with on-boarding/orientation of new staff;
  • Lead team in-service training on topics that improve service to clients;
  • Integrate medical, specialized, and behavioral health services with social support and educational services;
  • Attend daily team meetings, weekly supervisions, and mandatory trainings;
  • Demonstrate exceptional communication skills and engage clients effectively in diverse settings.

Requirements:

  • Master's Degree in Human Services or a related field with appropriate NYS licensure;
  • At least one year of experience working on or with ACT teams or similar community-based programs;
  • Multi-lingual preferred, with ability to speak, write, and understand English;
  • Comfortable with public transportation and traveling in the community, with 80% time spent in the field and 20% in the office;
  • Ability to walk and stand for long periods.
