About the Role

We are seeking an experienced Registered Nurse to join our team as a Primary Care RN Consultant/Case Manager. This role is responsible for providing direct care coordination, ensuring the care delivered is effective and efficient, and achieves desired patient outcomes.

Key Responsibilities
  • Provide clinical nursing care to clients according to established professional nursing practice standards of the American Nursing Association and in accordance with the North Carolina Nurse Practice Act and other regulatory standards.
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
  • Develop and implement individualized care plans that address the unique needs of each patient.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal care.
Requirements
  • Graduate of an accredited BSN nursing program.
  • 1 year of nursing experience in primary care and/or public health setting preferred.
  • Evidence of excellent oral and written communication, high level of current clinical knowledge.
  • Basic PC skills including Windows Operating System and Electronic Health Records. Typing skills of 30 WPM.
  • Bilingual preferred, but not required.
  • Certification/Licensure: RN currently licensed to practice in North Carolina.
