Job Title: Diabetes Educator

Job Summary:

We are seeking a highly skilled Diabetes Educator to join our team at Northwell Health. The successful candidate will be responsible for planning and conducting nutritional programs to promote health and control disease.

Key Responsibilities:

  • Assess nutritional needs and provide individualized education to patients, their families, and/or significant others.
  • Develop and implement nutritional programs to promote health and control disease.
  • Communicate patient information to referring providers and appropriate staff in a timely manner.
  • Document nutrition information discussed, written materials provided, and evaluation of patient's receptiveness and comprehension of the education provided.

Requirements:

  • Bachelor's Degree in Food, Nutrition, or Dietetics.
  • Registered Dietitian certification.
  • Current Diabetes Educator Certification.
  • Minimum of three years progressively responsible, related experience.
