Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team of healthcare professionals. As an LPN with Pinnacle Home Care Inc., you will play a vital role in providing personalized care to our patients, working closely with our interdisciplinary team to develop and implement individualized care plans.

Key Responsibilities
  • Provide Skilled Nursing Care: Deliver high-quality nursing care to patients in their homes, under the supervision of a Registered Nurse.
  • Assess Patient Needs: Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted care plans.
  • Implement Care Plans: Work with patients and their families to implement care plans, providing education and support to promote optimal health outcomes.
  • Monitor Patient Progress: Regularly monitor patients' progress, making adjustments to care plans as needed to ensure optimal outcomes.
  • Collaborate with Interdisciplinary Team: Work closely with our interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care and coordination.
  • Document Patient Information: Accurately document patient information, including medical history, treatment plans, and progress notes.
  • Maintain Patient Confidentiality: Ensure the confidentiality and security of patient information, adhering to all relevant laws and regulations.
Requirements
  • Education: Graduation from an accredited School of Practical Nursing.
  • Licensure: Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.
  • Experience: One or more years of experience in an acute/sub-acute care facility, with a preference for home health experience.
  • Certifications: IV Certification, BLS, and LPN licensure.
  • Skills: Wound care and ability to work in a fast-paced environment.
