Position Summary

The Licensed Practical Nurse (LPN) plays a vital role in ensuring the overall health and comfort of residents by delivering a comprehensive range of services tailored to their unique needs. Reporting directly to the Health & Wellness Director, the LPN is responsible for overseeing the care provided by Certified Nursing Assistants (CNAs) and Resident Assistants (RAs). This position requires respectful and compassionate interaction with residents, their families, visitors, and fellow staff members. It is essential to note that this role is safety-sensitive.

Key Responsibilities

While the following duties are essential, it is understood that additional tasks may be assigned as necessary:
  • Facilitate the admission process and guide new residents through their transition into the community.
  • Regularly monitor and document residents' health and mental status, promptly reporting any changes to the attending physician.
  • Administer prescribed treatments and document residents' responses accurately.
  • Collaborate with physicians regarding medication management and work with pharmacy staff to ensure proper handling and disposal of medications.
  • Uphold and protect all resident rights as outlined in the Resident Bill of Rights.
  • Implement infection control protocols and ensure compliance among staff.
  • Work closely with dietary personnel to guarantee that residents receive appropriate nutritional and therapeutic meals.
  • Investigate and document any incidents or concerns involving residents.
  • Coordinate the discharge and transfer processes for residents.
  • Provide direct care and support to residents as needed.
  • Assist in training new staff members.
  • Manage responsibilities under tight deadlines and in potentially high-pressure situations.
  • Engage in daily resident programming activities.
  • Maintain consistent and reliable attendance as a fundamental aspect of this role.
Essential Skills and Physical Requirements

The following skills and physical capabilities are necessary for successful performance in this position:
  • Proficient in reading, writing, and following both written and verbal instructions in English.
  • Ability to think critically and act appropriately based on detailed information.
  • Capable of walking long distances regularly.
  • Ability to lift and carry items weighing up to 50 lbs.
  • Skilled in pushing wheelchairs with residents weighing up to 200 lbs.
  • Repetitive hand motions are required.
  • Ability to hear and engage in ordinary conversations and office sounds.
  • Effective verbal communication skills, both in person and over the phone.
  • Competence in written communication, both by hand and on a computer.
Qualifications

Minimum Requirements
  • Valid Florida LPN license; must maintain licensure.
  • Completion of an LPN Supervisory Course or willingness to complete within six months of hire.
  • Two or more years of relevant experience in geriatrics, long-term care, home health, or similar healthcare settings.
  • Exceptional verbal and written communication abilities.
  • Proficiency in computer applications, including Microsoft Office Suite.
  • Strong interpersonal and communication skills.
