Job Overview

In an outpatient setting, the Advanced Practice Nurse provides client-focused, evidence-based care to diverse individuals and their support systems, maximizing health, well-being, functionality, and resiliency. The APN provides a process of patient care including assessment, diagnosis, treatment planning, intervention, and evaluation of outcomes. Collaboration with other healthcare providers, internally and externally, as well as community and social service organizations is routine.

Key Responsibilities
  • Provide psychiatric evaluations, SBIRT, and motivational interviews utilizing diagnostic criteria based on the DSM-5-TR.
  • Educate patients and families about treatment plans, interventions, medications, mental illness, symptom relief, comorbidities, and strategies to enhance overall health.
  • Provide medication management based on target symptoms, psychopharmacological evidence, Federal & State Regulations.
  • Complete written evaluations, medical records, progress notes, and service coding using an EMR system (familiarity with Epic is a plus).
  • Maintain confidentiality of patient treatment and records in accordance with HIPAA.
Qualification Requirements
  • A Master's or DNP degree with a PMHNP concentration and board certification by the American Nurses Credentialing Center (ANCC).
  • Current State of New Jersey APN license, CDS, DEA, and BLS, or ability to obtain prior to starting employment.
  • Three to five years of relevant work experience preferred.
  • A track record of success working in an environment of collaboration, transparency, mutual respect, and shared passion for excellence and innovation.
  • The ability to express oneself clearly and concisely both orally and in writing.
  • The ability to plan, organize, prioritize daily tasks, and work with minimal supervision.
