Treatment Program Overview:

Time Wellness offers an evidence-based treatment program that focuses on providing clients with the tools they need to succeed. Our team of experienced professionals is dedicated to delivering high-quality care in a supportive environment.

Job Description:

We are seeking a skilled Case Manager to join our team. The ideal candidate will have experience working with clients with mental health and substance abuse issues and be able to develop and implement effective treatment plans.

Key Responsibilities:

  • Develop and implement individualized treatment plans within the first 48 hours of client admission.
  • Coordinate with therapists to address adjunct therapy issues.
  • Facilitate clients' access to medical appointments.
  • Submit necessary paperwork to support employment and benefits for clients.
  • Communicate with legal contacts to facilitate discharge planning.
  • Set up aftercare plans based on clinical recommendations.

Requirements:

  • High School Diploma or equivalent (required).
  • Bachelor's Degree in a related field (preferred).
  • Minimum one year experience in healthcare, preferably with mental health and substance abuse populations.

Salary Range:$50,000 - $65,000 per annum, depending on experience.
