Social Educator

Job Summary:

Fusion Dynamics is seeking a highly skilled and compassionate Social Educator to join our team in the ambulatory sector. As a Social Educator, you will play a vital role in supporting individuals with disabilities in living independently and promoting self-determination.

Key Responsibilities:

• Provide support and guidance to individuals with disabilities in daily life, promoting independence and self-advocacy.
• Develop and implement personalized support plans to meet the unique needs of each individual.
• Collaborate with legal guardians, family members, healthcare professionals, and other stakeholders to ensure comprehensive support.
• Conduct regular assessments and evaluations to monitor progress and make adjustments as needed.
• Maintain accurate and detailed records of individual progress and support plans.
• Participate in ongoing training and professional development to stay up-to-date on best practices and industry developments.

Requirements:

• Completed degree in Social Pedagogy or a related field.
• Proven experience working with adults with disabilities, preferably in an ambulatory setting.
• Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals, families, and healthcare professionals.
• Ability to work independently and as part of a team, with a strong commitment to collaboration and mutual support.
• Valid driver's license and access to a reliable vehicle.
