Job Title: Registered Nurse (RN) - Hospice On Call

BAYADA Home Health Care is seeking a skilled Registered Nurse (RN) to join our team as a Hospice On Call nurse. This is a unique opportunity to provide high-quality care to clients in their homes, while enjoying a great work-life balance.

Job Summary:

  • Provide comprehensive nursing care to clients in their homes, focusing on palliative care and symptom management.
  • Work independently, managing time effectively and prioritizing tasks to ensure timely completion of care plans.
  • Collaborate with the multidisciplinary healthcare team to develop and implement individualized care plans.
  • Conduct thorough assessments of clients' physical and emotional needs, identifying areas for improvement and developing strategies for care.
  • Administer medications, perform wound care, and provide other treatments as prescribed by physicians.
  • Document all interactions, assessments, and interventions in a timely and accurate manner.

Requirements:

  • Current licensure as a Registered Nurse in Vermont.
  • Minimum one year of recent, verifiable clinical experience.
  • Prior home care experience is strongly preferred, but not required.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal skills and ability to communicate effectively with clients, families, and healthcare team members.
  • Solid computer skills and prior experience with electronic medical records (EMR) is preferred.
