Position Overview:

As a Licensed Practical Nurse (LPN) II, you will provide essential nursing care to a designated group of patients, following physician directives, the nursing care plan, and the guidelines set forth by the Oklahoma Board of Nursing.

You will collaborate with registered nurses to devise effective patient care strategies and perform a range of direct and indirect patient care tasks, including thorough data collection.

Educational Requirements:

Completion of a state-approved Licensed Practical Nurse program or equivalent registered nurse coursework recognized by the Board of Nursing.



Licensure and Certification:
A valid LPN license issued by the State of Oklahoma is required.

Experience: 0 - 6 months of relevant experience is preferred.

Skills and Competencies:
A solid understanding of general disease management processes, the bio-psychosocial model, and clinical pathways is essential. You should possess effective communication skills, both written and verbal, along with the ability to organize and prioritize tasks efficiently. Attention to detail is crucial, especially when reviewing clinical and numerical data. Proficiency in IV therapy medication administration, in accordance with hospital policies, is also required.

Key Responsibilities:
- Execute nursing skills appropriate for the patient demographic.
- Demonstrate a comprehensive knowledge base relevant to your specialty area, considering the age and bio-psychosocial needs of patients.
- Collect data, implement, and assess care for assigned patients based on physician orders and the Nursing Care Plan.
- Accurately document physician orders and collaborate with Registered Nurses to strategize patient care.
- Maintain precise records of nursing processes and care.
- Address patient issues and monitor progress towards established outcomes.
- Facilitate patient education and assist in discharge planning.
- Make necessary referrals and adhere to care plans, taking into account patient age and cultural/psychosocial factors.
- Contribute to unit and hospital objectives through performance improvement and educational initiatives.
- Attend mandatory training sessions and meet educational standards for the unit.
- Complete variance reports accurately and adjust practices to align with care standards and unit expectations.

Decision-Making:
You will be responsible for executing non-routine procedures in a dynamic environment, following general instructions from your supervisor.

Collaborative Relationships:
You will lead peers in similar tasks (without direct supervision) and engage directly with patients and/or customers. Interaction with internal and external clients will occur via phone or face-to-face communication, collaborating with other healthcare professionals and staff.
