We are seeking a skilled and compassionate RN Case Manager to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in delivering high-quality patient care and support to individuals in their homes.
Key Responsibilities- Provide comprehensive case management services to patients, including admission, ongoing care, and follow-up visits.
- Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans.
- Conduct thorough assessments of patients' needs and develop plans to address their physical, emotional, and social well-being.
- Coordinate care with other healthcare providers, including physical therapists, occupational therapists, and home health aides.
- Ensure accurate and timely documentation of patient care and progress.
- Diploma, Associate, or Bachelor Degree in Nursing.
- Minimum of one year of nursing experience, preferably in home health or a related field.
- Strong med-surg, ICU, ER, or acute experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Strong organizational and communication skills.
- A valid driver's license, auto insurance, and reliable transportation.
