HealthEcareers - Client is seeking an LPN (Licensed Practical Nurse) to provide high-quality care and support to residents in our Rushford, Minnesota facility.
About the RoleThis position requires a compassionate and skilled nurse who can work independently and as part of a team to deliver exceptional patient care. As an LPN, you will be responsible for assessing resident needs, developing and implementing individualized care plans, and providing direct nursing care. Your goal will be to enhance the quality of life for our residents and promote their independence and dignity.
Responsibilities- Conduct thorough assessments of residents to identify their medical, emotional, and social needs
- Develop and implement individualized care plans that address resident goals and objectives
- Provide direct nursing care, including administering medications, performing treatments, and documenting resident progress
- Collaborate with other healthcare professionals to ensure comprehensive care and coordination
- Communicate effectively with residents, families, and staff to ensure seamless care transitions and informed decision-making
To be successful in this role, you must have a graduate degree from a licensed practical nurse accredited program and hold an active LPN license in the State of Minnesota. Additionally, you should have current CPR certification and be able to read, write, and communicate in English. Gerontological nursing experience and knowledge of physical assessment techniques are highly desirable.
