Home Health Registered Nurse Opportunity

Key Responsibilities:

  • Provide high-quality home-based care to patients with various health conditions
  • Work collaboratively with a team of healthcare professionals to ensure comprehensive patient care
  • Conduct thorough assessments, develop care plans, and educate patients and their families on disease management and self-care
  • Coordinate with physicians and other healthcare providers to ensure seamless care transitions
  • Ensure patient safety and well-being through regular monitoring and reporting

Requirements:

  • Active Registered Nurse (RN) License in Colorado
  • Home health care experience preferred but not required
  • No Case Manager responsibilities
  • Graduate of an accredited school of nursing
  • At least 1200 hours of experience as an RN
  • Healthcare CPR/BLS Certified
  • Pass a background check and drug test
