We are seeking a highly skilled and compassionate Patient Care Manager to join our team at LHC Group. As a Patient Care Manager, you will play a critical role in ensuring the delivery of high-quality, patient-centered care in the home health setting.
Key Responsibilities- Supervise and Coordinate Clinical Services: Oversee the provision of nursing care to patients in their homes, ensuring that care is delivered in accordance with physician orders and applicable laws and regulations.
- Provide Clinical Leadership: Lead a team of clinicians in the delivery of patient care, providing guidance and support as needed to ensure that patients receive the highest quality care.
- Coordinate Patient Care: Receive referrals, ensure appropriate clinician assignments, evaluate patient orders, and plot start of care visits to ensure that patients receive timely and effective care.
- Ensure Patient Needs are Met: Continuously assess patient needs and ensure that care rendered is individualized to patient needs, appropriate, and reasonable, and meets home health eligibility criteria.
- Review and Revise Care Plans: Review assessments and plans of care daily, per assigned workflow, and consult with clinicians with recommendations as needed.
- Current RN Licensure: Must be currently licensed as a Registered Nurse in the state of practice.
- CPR Certification: Must hold current CPR certification.
- Driver's License and Vehicle Insurance: Must hold a current driver's license, have vehicle insurance, and have access to a dependable vehicle or public transportation.
