Job Overview:

The Pharmacist Senior Transitions of Care assumes a key role in providing drug information and support to healthcare teams, focusing on patient populations and age groups.

Key Responsibilities:
  • Participate in patient care rounds on assigned teams to ensure comprehensive care.
  • Provide verbal consultations on drug therapy evaluations and recommendations.
  • Deliver focused pharmaceutical care through initial patient assessments and medication reconciliation to enhance patient medication adherence.
  • Offer individualized patient education and evaluation on drug therapy as requested or identified.
  • Evaluate necessary laboratory tests to monitor and support patient drug therapy regimens.
  • Address allergy and medication reconciliation consults for assigned teams.
  • Represent the Pharmacy Services department on committees as assigned by management.
  • Enforce institutional policies and procedures related to medication ordering, dispensing, storage, and administration.
  • Supervise and assist pharmacy technicians and other support staff effectively.
  • Participate in the design and update of the Electronic Health Record (EHR) related to pharmacy.
  • Develop and participate in training and mentorship programs for pharmacists, pharmacy students, residents, and technicians.
  • Serve as a preceptor for pharmacy students and/or residents.
  • Maintain professional competency to meet current and future needs of the facility.
  • Comply with facility policy and procedures and regulatory requirements.
  • Perform other duties as assigned.
