Job Overview
Under the guidance of the assistant director of Transition and Vocational Services, the Transition Specialist in Special Education is tasked with ensuring the effective application of the Self-Determination Learning Model of Instruction (SDLMI) or SDLMI-Transition with Mentoring for students participating in the grant-funded initiative.

The SDLMI is a research-supported program designed to assist students in developing self-determination skills, which are crucial for post-school achievement among individuals with disabilities. The SDLMI Transition framework empowers students to learn self-regulation in setting transition-oriented objectives, devising strategies to achieve these goals, and assessing their progress.

This role includes a mentoring component aimed at helping students formulate post-school aspirations, navigate community resources, acquire essential transition competencies, and engage in significant transition activities aligned with their identified post-school objectives.

Job Requirements
The following qualifications are mandatory:
  1. High school diploma or GED.
  2. A minimum of five years of experience in special education or a related domain. Relevant education and/or training may substitute for up to four years of work experience.
  3. Experience in mentoring individuals with disabilities.
  4. Experience collaborating with bicultural and bilingual families.
  5. Ability to establish and maintain communication with pertinent school, family, and community organizations.
  6. Ability to coordinate meetings and activities effectively.
  7. Knowledge of record-keeping practices and data collection skills.
  8. Ability to foster and maintain productive working relationships.
  9. Proficient computer and organizational skills.
  10. Crisis prevention and intervention capabilities.
  11. Commitment to confidentiality.
  12. A valid driver's license and personal transportation for home visits and community outreach are required.
Preferred Qualifications
The following qualifications are desirable:
  1. An associate's or bachelor's degree in special education, social services, counseling, or a related field.
  2. Understanding of the challenges faced by youth and families with disabilities and low socioeconomic status.
  3. Familiarity with community resources available to assist youth and families.
  4. Ability to engage with parents, students, educators, and administrators in a supportive and professional manner.
  5. Knowledge and experience regarding transition processes and services for students with disabilities.
  6. Experience working with youth in various settings.
  7. Experience in delivering evidence-based interventions and participating in research initiatives.
Essential Job Functions
  1. Participate in training sessions as required.
  2. Coordinate with external partners to schedule and complete implementation activities.
  3. Prepare and deliver instructional materials for small group and one-on-one mentoring sessions.
  4. Communicate with families to support transition planning activities.
  5. Collaborate with IEP team members to enhance educational opportunities related to transition planning.
  6. Engage directly with students on the SDLMI curriculum.
  7. Collect, analyze, and interpret data to inform instructional practices.
  8. Provide case managers with updated information essential for educational support and outcomes.
Physical and Mental Demands
The physical demands of this position include frequent standing, walking, sitting, speaking, and hearing. Specific visual abilities are also necessary. The employee is regularly required to reach with hands and arms and occasionally lift items weighing less than 40 lbs. Additionally, the employee must be capable of understanding vague and implicit instructions, recalling facts and details, managing conflict, and making effective decisions under pressure. The role requires the ability to handle the stress of working with diverse individuals in a dynamic environment.

Work Environment
The work is conducted in a professional setting with a diverse range of individuals possessing varying functions, personalities, and abilities. The employee may be exposed to outdoor weather conditions and will frequently interact with a video display. The noise level in the work environment is typically moderate.
