Job Title: Community Health

The Community Health Worker plays a vital role in our multidisciplinary team, providing support to participants to help them achieve good health practices around cardiovascular disease. Key responsibilities include facilitating health improvement, health promotion, health program development, disease prevention activities, and coordination with community partners.

Key Responsibilities:
  • Facilitate health improvement and health promotion activities for participants
  • Develop and implement health programs to prevent disease and promote healthy behaviors
  • Coordinate with community partners to provide referrals for health information and services
  • Act as a health coach, providing one-on-one education on cardiovascular disease risk and encouraging participants to achieve desired health behaviors
  • Assist participants with navigation of the healthcare system and processes
  • Speak with diverse populations about cardiovascular health risk in a sensitive and culturally competent manner
  • Advocate for participants to ensure healthcare program compliance
  • Provide participants with community resources as needed
  • Maintain data and files for participant records and program reporting
Requirements:
  • High School Diploma or Equivalent
  • 1 year of health/clinical experience or equivalent health-related education
  • Bachelor's Degree
  • Research and/or data collection experience

Background Check, Physical, and Drug Screening: Selected candidates must complete and pass a background check and an occupational health screening as a condition of employment. For identified jobs, a drug screening will also be required.
