Care Coordination Specialist

Job SummaryWe are seeking a highly skilled Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in facilitating multiple components of patient care, including gap closure, data entry, and scheduling appointments with medical providers. You will also perform telephonic outreach, educate patients and families about preventive health screenings, and report any possible issues related to member healthcare.
Key Responsibilities- Facilitate gap closure and ensure timely and accurate data entry into the designated care management module.
- Schedule appointments with medical providers to access needed preventive/follow-up care services and close gaps in care.
- Inform providers about care gaps and schedule visits accordingly.
- Perform telephonic outreach to patients in need of preventive care services and follow up care.
- Educate patients and families about preventive health screenings and identify barriers to care.
- Report any possible issues related to member healthcare to the appropriate stakeholder.
- Facilitate chart/record submission, portal data entry, and supplemental data submission to various payers as required.
- Assist with care navigation and complete proactive outreach and engagement for patients with open care gaps.
- Generate reports as required and work in compliance with federal, state, and local regulations.
- High school diploma or equivalent.
- Experience with electronic health records and comfort using technology for virtual communication, scheduling, and documentation.
- Excellent customer service skills and ability to interact with people at all levels of the healthcare system.
- 2+ years of experience in a healthcare delivery setting, preferably communicating with medical providers, healthcare customer service, or call center experience.
- Medical terminology and fluency in Cantonese, Mandarin, Ilocano, Chuukese, Marshallese, or Tagalog.
