We are seeking a compassionate and experienced Hospice Nurse to join our team as a RN Case Manager.

About the Role

As a RN Case Manager, you will be responsible for coordinating and managing the care of terminally ill patients, collaborating with our interdisciplinary team to develop and implement individualized plans of care, and ensuring seamless transitions between levels of care. Your expertise in assessing patient needs, creating care plans, and communicating effectively with families will make a meaningful difference in the lives of those we serve.

Responsibilities
  • Perform thorough patient assessments and create comprehensive care plans tailored to individual needs
  • Collaborate with our interdisciplinary team to ensure cohesive and effective care delivery
  • Communicate regularly with patients, families, and caregivers to address concerns and provide updates on patient status
  • Maintain accurate and timely documentation of patient progress and care provided
  • Participate in case conferences and other meetings to share knowledge and best practices
Requirements
  • 1+ year(s) of nursing experience in acute care setting(s), ER or fast-paced environment(s)
  • Current RN license, specific to the state(s) you are assigned to work
  • Current CPR certification
  • Valid driver's license, reliable transportation and liability insurance
  • HCHB and/or previous hospice experience a HUGE plus
