Responsibilities:
  • Provide comprehensive nursing care to patients in their homes, including administering medications, wound care, and vital sign monitoring.
  • Collaborate with our healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Participate in ongoing education and training to stay current with best practices and advancements in home healthcare.
  • Contribute to a positive and supportive team environment by promoting a culture of respect, empathy, and open communication.
Requirements:
  • Graduation from an accredited Licensed Practical Nursing program.
  • Current LPN licensure in the state where you will be practicing.
  • 1 year of experience as an LPN, preferably in a home healthcare setting.
  • Ability to work independently and as part of a team to achieve patient-centered goals.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
