Job Title: Home Health Speech Language Pathologist

VNA Health Group is seeking a skilled Speech Language Pathologist to join our team in providing high-quality care to patients in their homes or community settings.

Job Summary:

We are looking for a dedicated and compassionate Speech Language Pathologist to assess and evaluate clients for speech and language therapy needs, develop and implement appropriate plans of action, and provide counseling and teaching strategies to promote optimal communication and well-being.

Responsibilities:
  • Assess and evaluate clients for speech and language therapy needs and develop and implement appropriate plans of action in collaboration with the physician.
  • Provide speech and language therapy evaluation and treatment to patients in their homes or community settings.
  • Document all changes in client's condition and update care plans and outcomes accordingly.
  • Identify counseling needs and plan appropriate teaching strategies.
  • Monitor quality and appropriateness of services provided.
  • Adhere to agency infection control and safety policies.
  • Adhere to the organization's policy in regards to absenteeism and appearance.
Qualifications:
  • Licensed Speech-Language pathologist with CCC-SLP certification and Master's degree in Speech-Language Pathology.
  • 2-3 years of prior clinical experience and community-based experience preferred.
  • Strong computer skills.
  • Strong communication and interpersonal skills.
  • Valid NJ drivers license.
